What types of private health insurance are available in Spain?

With surcharges (con copagos)

The patient pays a small fixed amount for each medical service, such as a doctor's visit, tests, examinations, etc.

Without surcharges (sin copagos).

All medical services are fully covered by insurance, with no additional costs to the patient.

With reimbursement (con rembolso)​

Insurance allows a patient to contact any doctor or medical center, pay for services themselves, and then submit documents for reimbursement.

When choosing health insurance in Spain, you may often encounter the concept of copago.

Copago is a co-payment for each use of medical services, in addition to the monthly payment for the policy.

How does it work?
Every time you visit a doctor, have tests, or receive medical advice, you pay a small fixed amount. The amount depends on the insurance package.
For example:
• Minimum copay — you pay a minimum amount for each visit to the doctor. (On average, it is 2-3 euros)
• Average copay — slightly higher copay, but monthly payments are lower.
• No copay — there are no copays for medical services, but the monthly payment will be higher.

Insurance with Rembolso (reimbursement) — is a type of health insurance that allows the insured person to contact any doctor or medical institution, even if they are not included in the insurance company's medical network. After receiving the service, the client pays for it himself, and the insurance company reimburses a certain percentage of the costs, according to the terms of the contract. Usually it is 80-100%, depending on the terms of the policy.

The Periodo de Carencia is a waiting period from the moment the insurance contract is concluded, during which certain services are not covered by the insurance. This means that even if the policy is already in force, access to certain types of medical care will only be possible after this period has ended.

What services does it apply to?
These are usually complex and expensive procedures, such as: surgeries, hospitalization, childbirth, and complex diagnostic tests.

Why is a Waiting Period needed?
It is used by insurance companies to prevent abuse, where a customer takes out a policy solely for the purpose of covering a planned medical procedure or surgery.

Exceptions:
In the case of emergency care, the waiting period is usually waived.
For those who switch from another insurance company to DKV, the waiting period is waived, with the exception of childbirth.

Discounts depending on the payment method: 2.8% (quarterly payment), 4.6% (semi-annual payment) and 7% (annual payment).

Basic insurance regulations in Spain

The insurance system in Spain consists of compulsory and voluntary insurance and is regulated by the Insurance Law, namely Ley 50/1980, de 8 de octubre, de Contrato de Seguro (Insurance Contract Law) and Ley 20/2015, de 14 de julio, de Ordenación, Supervisión y Solvencia de las Entidades Aseguradoras y Reaseguradoras (Law on the Management, Supervision and Solvency of Insurance and Reinsurance Companies).

Duration of the insurance contract  

When concluding an insurance contract in Spain, the contract is usually concluded for a period of 1 year with subsequent automatic renewal. The exception is travel insurance, which is often valid for a short period.
